Dear Volunteer: In order to ensure that we are protecting the children and vulnerable individuals entrusted to our care, the Archdiocese of Atlanta has established procedures that must be consistently adhered to by all of our parishes, schools and agencies. This letter will explain these procedures and address concerns related to the process. All volunteers of the Archdiocese of Atlanta should complete the Safe Environment Forms. Thiese forms include some basic information and references. Any volunteer who has regular contact with children or vulnerable individuals must complete the Safe Environment forms. Please notify Penny Fitzpatrick, pfitzpatrick@transfiguration.com, if you are serving in PREP and Linda Rega, lrega@transfiguration.com, if you are volunteering with another group comprised of children or vulnerable adults. They will email you an evite to complete the forms electronically. If you prefer, you can download and complete the forms below. Adult Medical Consent Form (only needs to be completed if you are chaperoning an overnight trip or sports related activity) Forms are also available here: http://archatl.com/offices/ocyp/senvironment/volunteers.html The volunteer background investigation includes a national criminal check for felonies and misdemeanors, verification that the social security number was validly issued to the individual and a check of the sexual abuse registry of 39 states. A national provider of background investigations performs all investigations on behalf of the Archdiocese. They first verify the addresses listed on the consent form to ensure that a potential volunteer has not left off addresses he or she may have lived at previously. This is an important step in the process because certainly a sexual predator who may have been caught in one jurisdiction would leave off that address information. Each time information is accessed for background investigation purposes a record is entered of the company who checked this information, so if you check your records with a consumer reporting agency you will see an instance of inquiry made on behalf of the Archdiocese. However, again be assured that credit is never checked for any volunteer by the Archdiocese. Also, please be assured that the agency that performs the backgournd check does not, under any circumstances, divulge to a third party the contents of any data produced, procedures used, work performed or results obtained as part of their work for the Archdiocese. Procedures have been established to ensure that at all times background investigation information is maintained with strict confidentiality at the parish, school and Archdiocesan levels. Information must be kept in locked file cabinets and access is not available to anyone other than those directly involved in processing the application. I hope this explanation will help you to understand the procedures in place with regard to background investigation for volunteers.
