2011 Tax Statements

Following IRS guidelines, we will mail statements to those contributing $250 or more at one time (one check, one credit charge, etc.).  Tax statements for all other contributors will be prepared by request only.

Request 2011 tax statements from Cindy Zondlak in the Records Office by submitting the form below or via:

 We will need the following information: 

• Envelope ID number (if known)
• First and last name(s)
• Address
• Contact phone number
• Specify if you want to receive your statement by mail or by email
• Email (if specified above) 
• Only contributions received in our office by December 31, 2011 will be included on the statements. 
• Contributions to the Peter’s Pence collection are not tax deductible and will not be included. 
• Contributions to the St. Vincent de Paul Society, A Place at the Table (Philippine outreach), and the Archdiocesan Annual Appeal will not be included.  These organizations have their own policies for issuing donation statements.
